The Department of Family Medicine Primary Care Research Institute at the Jacobs School of Medicine and Biomedical Sciences has an exciting opportunity for a Case Manager. As a Case Manager, you will be responsible for case management and community outreach with criminal justice-involved or formerly criminal justice-involved individuals with co-occurring substance use and mental health problems in the Critical Time Intervention, using the MISSION-CJ (Maintaining Independence and Sobriety through Systems Integration, Outreach, and Networking - Criminal Justice Edition) evidence-based model.

 

	Minimum Qualifications   Bachelor’s degree in Psychology, Social Work or related field, in conjunction with two years of relevant experience.
	Preferred Qualifications CASAC, experienced with opioid addiction. Familiarity with trauma-informed care. Strong case management experience
